Non US Citizen

If you are a citizen of a country other than the US. You must get a passport from that government. Find out how to do it and if you can do it (some governments are reluctant to issue passports). Without a valid passport, you don't go.

US Citizen

For any foreign travel you will need a passport. This is issued by your government.

In the United States, you can get a passport application at your county court house, or some post offices or you can print your own from this government web site .

Fill out this application. Name, Address, Date of Birth, Place of Birth, Etc. This must be signed before a clerk of the court. (Parent signature required for minors.) This application, along with two color photos, and your birth certificate must be mailed to the US Government (Address on the application). I have heard the cost is $80 for an adult, good for 10 years, or $40 for a minor, good for 5 years. The sponsor will pay this if necessary (however, he will hold the passport until departure). The government can take up to six weeks to send you a passport.
